Licensed Clinical Social Worker (LCSW)

Location: Plaquemine, LA, and Donaldsonville, LA

We are seeking a compassionate and experienced Licensed Clinical Social Worker (LCSW) to join our Behavioral Health team. The LCSW will provide comprehensive clinical services, including assessments, individual, group, and family therapy, treatment follow-up, referrals, care coordination, and patient education.

The position will work with children, adolescents, families, and adults and may address complex behavioral health and social concerns, including abuse, neglect, domestic violence, and exposure to community violence.

• Conduct initial assessments, evaluations, diagnosis, treatment follow-up, and client education.

• Provide individual, group, and family therapy.

• Develop and implement appropriate treatment interventions and strategies.

• Monitor changes in patient symptoms and treatment needs.

• Coordinate referrals, support services, prior authorizations, and community resources.

• Maintain and update effective referral and resource lists.

• Collaborate with psychiatrists, the Medical Director, CEO, and other healthcare providers.

• Serve as an effective communication link between patients and providers.

• Advocate for patient safety, quality of care, and overall well-being.

• Provide education to clients and families regarding diagnoses and treatment.

• Consult with psychiatric providers regarding information relevant to medication management.

• Maintain accurate, timely, and appropriate clinical documentation.

• Prepare monthly productivity and performance reports.

• Assist with studies, analysis, and documentation related to patient care and departmental efficiency.

• Participate in required organizational meetings, committees, and in-service training.

• Utilize technology to improve efficiency and effectiveness of services.

• Maintain strict confidentiality in accordance with organizational policies and procedures.

• Participate in relevant professional and outside association activities.

• Perform other duties as assigned by the supervisor and/or Medical Director.

• Master's Degree in Social Work (MSW) from an approved and accredited institution.

• Active Licensed Clinical Social Worker (LCSW) license .

• 3-5 years of post-master's experience preferred.

• Clinical experience working with children, adolescents, families, and adults .

• Strong understanding of clinical assessment, treatment planning, therapy, documentation, and patient advocacy.

• Excellent communication, organizational, and interpersonal skills.

• Ability to organize, prioritize, and work independently.

• Ability to manage multiple responsibilities and complete work in a timely manner.

• Proficiency with computers, electronic systems, and standard software applications.

• High level of professional ethics and commitment to patient confidentiality.
